DLC’s Hospital Bridge Program brings peer support to overdose patients

by The 100 Companies

The Hospital Bridge Program is a collaboration between David Lawrence Centers, Physicians Regional Medical Center and NCH Healthcare System.

The program is designed to improve linking the hospital systems with DLC’s substance use programs and provide treatment, transitional support, certified recovery housing and other wrap‐around services to prevent readmissions.

Certified Peer Recovery Specialist Jake Ponsky, who battled an addiction to heroin, crack cocaine and alcohol, brings more than six years of recovery to his work. He supports the engagement, treatment and recovery of the individuals he visits in the emergency room who have suffered from overdose and substance use-related complications.
