Core Health Partners launches medical services at Grace Place Center for Children and Families

Core Health Partners and Grace Place Center for Children and Families have joined in a community-centered partnership. Core Health will provide developmental screenings, diabetes and nutritional education, and medical interventions through speech, physical and occupational therapy at the Grace Place campus.

Beyond the screenings, medical professionals will be on site to provide follow up treatment for children that need medical interventions such as diabetes education, behavioral therapy, occupational therapy, physical therapy and speech therapy. Treatments will be provided at the Grace Place campus, to ease the challenges of transportation, family and work commitments.
